CPAF Screening questionnaire 2021/22

Community pharmacy contractors will be able to complete their Community Pharmacy Assurance Framework (CPAF) screening questionnaire between Monday 28th June 2021 and midnight on Saturday 24th July 2021

The process involves completing an initial screening questionnaire consisting of 10 questions, which are a combination of repeated or amended questions from earlier years and some new questions.

The questionnaire needs to be submitted via the Manage Your Service (MYS) Portal, and pharmacy contractors should have received an email on how to complete the form either from the NHS Business Services Authority (NHSBSA) or from their own head office from the 21st June 2021.

If you have not received details of the questionnaire by Tuesday 29th June 2021, or would like to inform NHSBSA of any problems or queries in completing the questionnaire, it is advised you contact the NHSBSA by email at nhsbsa.cpaf@nhs.net.

All pharmacy contractors are required to complete the questionnaire as a requirement of the Terms of Service.  It also shows that the pharmacy is complying with the Terms of Service, and depending on the screening answers, could mean avoiding an unnecessary monitoring visit.
