Guidance issued to support pharmacy with hospital discharge MURs

Guidance has been issued to support pharmacy with targeted MURs for patients discharged from hospital.

This includes information on engaging with hospital pharmacy and some practical tools such as a patient referral form.

There is also guidance for hospital pharmacists and it is worth reading this document so you are aware of the advice they have been given on engaging with community.

It is worth noting that the guidance advises that where a patient has received their first prescription from a secondary care setting the community pharmacy can provide the patient with the NMS service, providing they meet the other criteria for NMS.

Although this guidance has been issued for England, Welsh members providing the DMR service may benefit from it.
